Getting started
Appointments
Are you accepting new patients?
The practice is welcoming patients. Availability can change, so call the office or submit an appointment request to confirm the appropriate visit type and next available option.
How do I request an appointment?
Use the Request an Appointment button or call (718) 689-7355. An online request is not a confirmed appointment; the office will follow up to confirm availability and next steps.
How long are appointments?
Visit length depends on the reason for the appointment and the type of care needed. The office will explain what to expect when your visit is confirmed.
What should I bring to my first visit?
The office can confirm what is needed for your appointment. In general, have identification, insurance information if applicable, a current medication list, and relevant records available. Do not send sensitive medical records through ordinary email.
Is an online request a confirmed appointment?
No. A request begins the conversation. Your appointment is confirmed only after the office contacts you with the date, time, visit setting, and any preparation instructions.

Visit Options
Do you offer office visits?
Yes. Office visits are by confirmed appointment at 73-12 170th Street, Ground Floor, Fresh Meadows, NY 11366. Call or submit an appointment request; the office will confirm availability and arrival instructions before you come in.
Do you offer telehealth visits?
Telehealth may be available when clinically appropriate and when an in-person examination is not required. The practice will confirm whether a virtual visit fits the concern.
Do you provide home visits?
Conditional cash-pay home visits may be available by request within New York City’s five boroughs: Queens, Manhattan, Brooklyn, the Bronx, and Staten Island. Home visits are not offered outside New York City. Availability depends on location, clinical fit, scheduling, and confirmation from the office.
How do I know which visit setting is right?
Start with a general appointment request or call the office. The practice will help determine whether office care, telehealth, a home visit, urgent care, or emergency care is the appropriate next step.

Insurance & Payment
Do you accept insurance?
Office visits accept Medicare, Medicaid, Fidelis, Empire Blue Cross Blue Shield, Cigna, GHI, EmblemHealth NYC PPO, and TRICARE East. Participation depends on your specific plan and visit. Call the office and your insurer to confirm network status, benefits, referrals, and patient responsibility before care.
How much do home visits cost?
House calls are cash-pay only: $350 per visit within Queens and $500 per visit in Manhattan, Brooklyn, the Bronx, or Staten Island. Home visits are limited to New York City’s five boroughs. The office confirms your location, clinical fit, availability, and what is included before accepting a visit.
Are home visits covered by insurance?
Home visits are cash-pay services and are not billed to insurance under this offering. Confirm the visit fee and what it includes with the office before scheduling.
Can you tell me what my plan will cover?
The office may provide general guidance, but your insurer determines benefits and patient responsibility. Contact the number on your insurance card for plan-specific confirmation.
